The Department of Infrastructure wants the public's view on the Island's roads, transport links, cycle routes and footpaths.
The questions are part of the UK's National Highways and Transport survey to compare 'satisfaction levels' to local authorities across the UK.
Over 3,000 surveys have been sent to Island residents and covers topics such as road condition and maintenance, bus services and accessibility.
The DOI ensures all responses are confidential.
