The Department of Health and Social Care will ask Tynwald for an extra £15.3 million when the court sits next week.
The supplementary vote is to cover extra costs from the past year, when Manx Care overspent on employee costs, off-island care and drugs inflation.
In March, Tynwald members voted to put off giving the department extra cash.
Health and Social Care Minister Claire Christian says her department needs to continue finding ways to work more efficiently and make the best use of public money.
