Manx Care’s paid out more than a million pounds in on-call and overtime payments so far this year.
Chief Minister Alf Cannan, who took over as Health and Social Care Minister after Lawrie Hooper resigned last month, was quizzed about the top earners at Manx Care.
In answer to a question for written reply, Mr Cannan said the top 20 were all medical staff and had been paid just under £3.4 million in the first six months of the year.
That included £1,170,161 for additional work, on-call and overtime.
