Tynwald will be asked to approve spending of over £18m to speed up surgery for 3,800 patients.
Health Minister Lawrie Hooper will ask for the funding to reduce waiting lists and enable Manx Care to continue its partnership with Synaptik.
The funding will be considered in the October sitting of Tynwald, and would be separate to Manx Care's annual budget.
CEO of the health body Teresa Cope says this significant investment would allow them to work to address the backlog of elective surgery at pace.
