A report into the deaths of three men on a Manx-flagged cargo ship says all three entered the wrong hold, which had been left unlocked.
The incident happened on the bulk carrier Berge Mawson as it was loading coal at Bunyu Island Anchorage in Indonesia in June 2022.
Three stevedores went into the wrong hold and died because there wasn't enough oxygen.
The Marine Accident Investigation Branch has recommended more training for stevedores and better guidance for crew.
