 
                                    The death of an officer on a Manx-registered ship last year has seen an investigation make three safety recommendations.
The UK Marine Accident Investigation Branch investigated the death of the chief officer of the MV Teal Bay who died when he was hit by a mooring line in August last year in Russia.
The MAIB says the mooring arrangement was unsuitable for loading from alongside another vessel, an operation to move the vessel had insufficient planning and crew and a poor emergency response led to delays in medical professionals assessing the man at the scene.
In its recommendations, the organisation says the Island's ship registry should pass on the safety lessons to other ships on the register.
