The Department of Health and Social Care has applied for permission to build a new 60-bed care home in Douglas.
In a planning application, the department says it wants to build the new facility on the former Glenside site on Victoria Road.
Glenside shut in 2012 and was demolished three years later.
In its application, the department says the proposals include a day care centre for elderly residents.
