The top earner at Manx Care was paid a total of £451,980 before taxes over the past year.
Health Minister Claire Christian provided gross pay figures in response to a Tynwald question for written reply from Rushen MHK Juan Watterson.
The top three workers made over £400,000, while all others in the top 20 list earned over £300,000.
All are medical staff, and none are agency workers.
