Chief Minister Alf Cannan has revealed the top 20 earners at Manx Care were paid £7.6 million before taxes, benefits and other deductions last year.
He was quizzed on the figures by Rushen MHK Juan Watterson in a question for written reply.
Mr Cannan said in total, the top 20 earners have made just under £7.7 million last year - and in the last six months they'd earned £3,389,200.
The data shows last year two staff earned more than half a million pounds - the top-paid made almost £548,000, while the second-highest earned more than £523,000.
Mr Cannan added the 20 were medical staff.
