Tynwald has unanimously approved extending the UK's Immigration Health Charge to the Island.
It will mean people applying for visas will have to pay a fee to contribute towards NHS services to access them.
The surcharge is part of the UK's 2014 Immigration Act.
Previously, migrants to the Island had been exempt from paying it.
