Manx Care has launched a new Health Passport designed to help people share information about their health and care needs.
The organisation says the document will support children and adults with learning disabilities or neurodivergence.
It allows patients and families to record medical details, emergency treatment plans and how they'd prefer to be communicated with.
The Health Passport is available in both printable and digital formats and can be taken to appointments or hospital visits - you can find out more here.
