Lack of openness and trust, a culture of blame and due process not being followed properly are being attributed to problems at Noble’s Hospital.
A review into three areas of the hospital - theatre and anaesthetics, emergency department and critical care has found a number of failings.
Immediate risks were identified in two of the areas following a review by the West Midlands Quality Review Service
The Department of Health and Social Care has now promised a robust and comprehensive action plan to tackle outstanding issues.
