The deadline to have your say on GP services across the Island is fast approaching.
8,500 people were randomly selected to complete the survey - aiming to shape the development of future heatlh care.
People have until Friday, October 18 to send in their copy.
The survey itself asks patients what matters most to them, and what they think needs either improving or changing - including the likes of out of hour services and making an appointment.
The collated results will be analysed by Ipsos MORI and the full results of the survey will be published at the end of November.
